How to get help
Call the office to request placement on the waiting list, then complete the online Security Questionnaire using the reference number provided by staff. The patient, caregiver, family member, or hospital social worker must call at least once every four weeks to confirm continued need and keep the need date current. Placement is not guaranteed, and alternate lodging plans are recommended.
