How to get help
Submit the online complaint form, or call and select option 1 to request a paper form. Provide full complainant contact information, the relevant licensee or pharmacy, a concise account with names and dates, and prescription-label information when applicable. Anonymous complaints are not accepted.
Who this service helps
Anyone may file a complaint concerning a Texas-licensed pharmacy, pharmacist, pharmacist intern, or pharmacy technician. Customer-service concerns, rudeness, pricing, and billing disputes alone are outside the Board's jurisdiction.
