How to get help
Complete the required Board complaint form accurately and fully, then submit it by email, fax, or mail to the Investigations Division. Include information sufficient to establish jurisdiction and explain the allegation.

People reporting conduct by a dentist, dental hygienist, registered dental assistant, or dental laboratory within Board jurisdiction. Anonymous complaints are not accepted, and the Board does not have jurisdiction over fee disputes.
