Complete, sign, and date the Request for Hearing section of Form 1757, explain why the distribution or retention is disputed, attach supporting evidence, select in-person or telephone participation, and return the original to the local child support office handling the case at the address supplied on the case form.

A custodial parent who is a current or former Temporary Assistance for Needy Families recipient and is dissatisfied with the distribution explanation on Form 1756 or a Monthly Report of Support Collected.
