How to get help
Complete ECHO Coordinated Assessment, receive an Integral Care eligibility determination and referral to HATC, then complete the HATC voucher application and finalization process.

Applicants must meet the applicable HUD homelessness definition, be receiving Integral Care services, and be referred by Integral Care through ECHO Coordinated Assessment. The primary target population is people considered chronically homeless.
