Hospital to Home is a voluntary, 24-bed residential program with an estimated 90- to 180-day stay. Participants receive temporary housing, therapeutic and life-skills groups, peer support, counseling, medical evaluation, clothing, meals, and help applying for benefits and long-term housing.
