Adults age 18 or older in the Community Healthcore service area may request assessment; program enrollment depends on residence, serious mental illness criteria, clinical need, and service authorization.

Service area: Gregg, Harrison, Rusk, and Upshur counties within the assigned region; Community Healthcore also serves Bowie, Cass, Marion, Panola, and Red River counties.
