Call 800-772-5987 for a brief screening and telehealth intake appointment. The child and guardian must participate; requested documents may be emailed to [email protected].

Children and adolescents ages 3-17 in the service area whose screening and pre-admission assessment establish clinical and program eligibility.

About this service
Each eligible youth is assigned a case manager who helps the child or adolescent and caregivers gain access to necessary treatment, supports, and community services within a family-centered mental-health plan.
