Who this service helps
Adults age 18 or older with a serious mental illness diagnosis who also meet at least one published hospitalization criterion: two psychiatric hospitalizations in the past 90 days, four in the past two years, or one psychiatric hospitalization lasting more than 30 days within the past two years.

Service area: Dallas and Waxahachie, with services delivered in eligible clients' homes and communities.

Cost / fees
The organization accepts major insurance plans, Medicare, Medicaid, and CHIP plans; free, reduced-cost, or sliding-scale care may be available based on financial eligibility. No one is turned away based on ability to pay.
About this service
The ACT team includes psychiatrists, nurses, social workers, and employment specialists who help adults with serious mental illness remain stable at home and avoid psychiatric hospitalization. Support can address trauma, housing, medication, coping and social skills, employment, benefits applications, independent living, transportation, and family education.
