Current Metrocare clients may request a referral at their local clinic. Other prospective participants may use the Healthy Transitions contact form on the Metrocare website.
Who this service helps
Youth and young adults ages 15–21 who need support transitioning into adulthood, subject to program admission.

About this service
Metrocare's Healthy Transitions program provides individualized mental health treatment, education and employment support, housing help, substance-use services, and life-skills development. Participants can receive help with money management, job readiness, communication, and independent living from a multidisciplinary support team.
