Referrals may be sent to [email protected]. A team member schedules the intake at the participant's home or current location, including a hospital or correctional setting.

Metrocare's Forensic Assertive Community Team provides 24/7 community-based services for adults with repeated hospitalizations who are also involved with probation, parole, or another part of the criminal-justice system. Services include medication support, trauma-informed care, skills training, substance-use support, housing help, legal advocacy, and care coordination.
