How to get help
Complete Form SSA-545-BK with the work goal, required items or services, costs, steps, and timeline, then submit it to Social Security for PASS review. A self-employment goal requires a business plan. Social Security decides whether to approve the plan.
Who this service helps
The applicant must be disabled or blind, have a feasible work goal expected to reduce or eliminate reliance on SSI or SSDI, and have income other than SSI or resources to set aside. Social Security evaluates the written plan and proposed expenses.
