How to get help
Identify trusted supporters, decide which choices require assistance, discuss the role with each proposed supporter, complete a supported decision-making agreement substantially similar to the statutory form, complete any needed information-release authorization, and retain and share signed copies where useful.
Who this service helps
For an adult with a physical or mental impairment that substantially limits one or more major life activities who voluntarily wants help making specified decisions and can choose a trusted adult supporter. The supporter assists but does not receive authority to make decisions for the adult.
