How to get help
Prepare a written complaint using DWC Form-154, an email, or a letter; gather supporting documents; and submit by email, fax, mail, or in person at a DWC field office. Call the consumer hotline for help filing.

People reporting an alleged legal or rule violation by a Texas workers' compensation system participant. A disagreement between an insurance carrier and an injured employee about a claim decision is a dispute and uses the separate dispute-resolution process.
